休克的中西医结合治疗近况

近年来,中西医结合治疗各类休克取得了不少的进展,不仅对休克的病因病机和临床辨证进行了深入的探讨,而且在治疗上,也摸索出一些新的有效方药和进行了剂型改革,收到了较好的疗效,现将有关资料综述如下:一、辨病与辨证的结合休克的临床表现同中医的厥证与脱证颇为相似。厥证可分为热厥与寒厥:热毒猖獗,邪气内陷可发生热厥。寒邪过盛,阳气衰微,可发生寒厥。脱证可分为阳(气)脱(亡阳),阴(血)脱

Clinieat data related to methods and experiences in the treatment of shock with TCM-WM in the past few years are reviewed and reported as follows: 1.Correspondence between the differentiation of symptom-complexes in TCM and the diagnosis in WM.The perishing of Yang(collapse,loss of vital energy)mainly corresponds to anaphylactic,cardiogenie and neurogenic shock.The perishing of Yin (loss of vital essence)mainly corresponds to hemorrhagic,traumatic and septic shock.The perishing of both Yang and Yin (excessive loss of both vital essence and vital energy)is mostly identical with various late-stage shocks,or complicated by DIC. 2.Principles and methods of treatment.Clinically three principles of treament are applied. 1).The replenishment of vital energy and restoration of pulse-beat.The commonly used recipe is Sbeng Mai San (生脉散,Radix Ginseng,Radix Ophiopogonis and Fructus Schisandrae). 2).The restoration of Yang from collapse.The commomly used recipe is Shen Fu decoction(参附汤,Radix Ginseng and Radix Aconiti Praeparata). 3).The promotion of normal flow of vital energy,invigoration of blood circulation and restoration of pulse and heart beat.Injections of Fructus Aurantii lmmaturus,scopolamine,etc.are usually used. 3.Experiences of treating shock with TCM-WM.Clinical experiences suggest that the integration of TCM with WM gives better effects,bringing their speciality and advantages into full play.It is important to suit measures to specific conditions.In case of patients with slight shock or impending shock,it is desirable to treat with Chinese herbs.In patients with severe shock or with little response to vasoconstrictor for hypotension,it is more effective to treat shock with the TCM-WM.In patients with shock,on whom operations have to be carried out,the dual effect of antishock and anesthesia may be obtained with the application of Chinese anesthetic herbs.
