慢性肾功能衰竭中医治疗的体会——53例临床分析

我院近5年来收治慢性肾功能衰竭(简称慢性肾衰)53例,以中医辨证论治为主进行治疗,现介绍如下:一般资料53例慢性肾衰都是慢性肾脏疾病迁延不愈,同时具备下列条件者:(1)尿素氮>60mg%,或非蛋白氮>80mg%;(2)严重贫血;(3)伴有酸中毒;(4)有严重的尿毒症症状和体征。53例病人中,男性36例,女性17例。年

53 cases of chronic renal failure have been reported.Among them 41 cases(77.4%)were caused by cllronic glomerulonephritis,5 cases(9.4%)by chronic pyelitis,4 cases(7.5%)by polycystic kidney,2 cases(3.8%)by purpuric nephritis,and 1 case(1.9%)by rheumatoid arthritis.Among the 41 cases of chronic glomerulonephritis of pfimary discase,the most frequently occurring hypertension type amounted to 27 cases,the common type 10 cases,and the nephrosis type 4 cases. The patients were treated by a combination of TCM-WM,using western medicine according to symptoms and using Chinese medicinal herbs according to differential diagnosis of symptoms and signs.The latter results in four types:Deficiency of both Qi(vital energy)and Yin(vital essence)in both spleen and kidney,31 cases,treated with Shen Qi Di Huang mixture(参芪地黄合剂),Da Bu Yuan decoction(大补元煎),and decoction ofSheng Mai San(生脉散煎剂)with modifications;deficiency of vital energy of the spleen and kidney,11 cases,treated with Bu Zhong Yi Qi decoction(补中益气汤),Bao Yuan decoction(保元汤),Fu Zi Li Zhong decoction (附子理中汤),and Zhen Wu decoction(真武汤)with modifications;deficiency of Yin of the liver and kidney,6 cases,treated with Qi Ju Di Huang decoction(杞菊地黄汤),and Zhi Bai Di Huang decoction(知柏地黄汤) with modifications;deficiency in both Yin and Yang(vital function)of the kidney,5 cases,treated with Gui Fu Di Huang decoction(桂附地黄汤),and Ji Sheng Shen Qi decoction(济生肾气汤)with modifications.—all these belong to asthenia of healthy energy.When pathogenic faetors are prevailing,such as evil wetness,stagnated blood, dampness and heat,wind and heat,etc.it should be treated with discretion.The efficacy of treatment:7 cases were markedly improved,13 cases moderately improved,21 cases failure,and 12 cases death.
