中西医结合治疗急性梗阻性化脓性胆管炎211例临床分析

本文总结了该院10年来中西医结合治疗急性梗阻性化脓性胆管炎211例的经验,非手术、手术组的死亡率分别为4.54%、3.96%。其中44例进行急诊经皮肝穿刺胆管造影术,成功率为90.9%,8例同时作置管外引流术,均于6小时内纠正休克。讨论中对图像诊断的分类方法以及非手术与手术治疗适应症的选择和时机提出了作者的看法。

英文摘要:

Two hundred and eleven cases of acute obstructive suppurative cholangitis(AOSC)were admitted to our hospital during the period of 1973 to April 1983.All cases had Charcot’s syndrome,32.7% in shock, 15.7% in coma,0.9% with biliary bleeding. Forty one cases were diagnosed with percutaneous transhepatic cholangiography(PTC),thirty four cases were found to have extrahepatic duct obstruction,six cases with combined intra-extrahepatic duct obstruction,and one case with intrahepatic duct obstruction.Eight cases were treated with PTCD.The patients were treated first with chinese herbal decoction.One hundred and ten cases were treated according to the basic theories of traditional Chinese medicine without surgical intervention with 5 deaths,non- operative mortality being 4.5%.One hundred and one patients had undergone surgical operation,with a postoperative mortality of 3.96%(4 deaths).Whenever complete obstruction of the biliary duct is found in a AOSC patient through PTC,surgical operation should be done at once
