      本文观察了中医辨证论治对原发性甲状腺功能减退症的临床疗效,血清垂体-甲状腺轴激素和外周血淋巴细胞核 T3受体的变化。发现助阳温肾补气治疗,可使甲减患者症状减轻,血清 T3、FT3、T4、rT3浓度增高,TSH 浓度降低,淋巴细胞核 T3R MBC降低。提示中药奏效机理主要是改善残存甲状腺细胞功能,促进激素分泌;而核 T3R MBC下降可能是病情好转在细胞水平的标志之一。
英文摘要:
      Serum levels of hormones of pituitary-thyroid axis and nuclear T3 receptors(NT3R)in circulating lymphocytes were studies in three groups of hypothyroid patients:(1)Group Ⅰ,untreated (n=10);(2)Group Ⅱ,treated with TCM(n=5);(3)Group Ⅲ,treated with desiccated thyroid(n=10). After treatment with TCM for 2~3 months,the 5 patients of Group Ⅱ showed marked clinical improvement,the symptoms of deficiency of Kidney-Yang(阳)were replaced by those deficiency of Kidney-Qi(气).Serum concentrations of T4,T3 and rT3 increased respectively to 1.49,1.65 and 1.67 times than that of the pretreatment value.FT3 increased from 2.02±0.15 pg/ml to 2.74±0.28 pg/ml (P<0.01)reaching the lower limit of normal values,and TSH levels decreased from 155±30 to 98± 37μu/ml(P<0.05).The MBC of nuclear T3R in lymphocytes of Group Ⅱ was 6.06±0.60 fmol/μg DNA,significantly lower than that of Group Ⅰ(8.06±0.80 fmol/μg DNA,P<0.001)but higher than those of Group Ⅲ(4.34±0.72 fmol/μg DNA)and control group(3.89±0.61 fmol/μg DNA,P<0.001). The MBC of NT3 R were negatively correlated with serum levels of T4,T3 and rT3,and positively correlated with TSH concentrations.It was suggested that the therapeutic effect of TCM might result mainly from the improvement of the functions of residual thyroid cells and secondarily from promotion of T4-deiodination with production of more active T3.The decrease of MBC of NT3R after treatment might be a useful marker at the cellular level indicating the improvement of hypothyroidism.
