彩阶B超对胆系疾病辨证的研究

目的：探讨现代医学影像检测方法与胆系疾病中医辨证的关系。方法：用彩阶Ｂ超检查了２１６例胆系疾病患者，并做脂餐试验。结果：肝胆湿热型８４例（３８．８９％），胆囊径较大，张力较高，胆汁透声差；肝郁脾虚型３９例（１８．０６％），胆囊横径小，张力差，细长形，收缩率低；瘀血停着型１３例（６．０２％），胆囊体积偏小，囊壁明显增厚或不规整，收缩率多明显降低；肝气郁结型７１例（３２．８７％）与肝阴不足型９例（４．１７％），介于肝胆热型与肝郁脾虚型之间，尚未发现明显的特征，主要靠临床鉴别。结论：本研究使“望诊”深入到机体内部，可为胆系疾病的辨证提供客观依据。

To evaluate the relationship between modern medical detective method of imaging and Syndromedifferentiation of TCM. Methods: Two hundred and sixteen binary tract diseases patients were observed with colorscale B-ultrasonography and fat meal tests. ResultS: Eighty-four cases(38. 89%) of damp-heat type in the Livergallbladder among the 216 patients had widened gallbladder, higher tension and weak bile echo-penetration through,39 cases (18. 06%)of type of Liver-Qi stagnation and Spleen deficiency had small transection, hypotension, slendertype and low contraction rate, 13 cases (6.02%) of Blood-stasis type had small gallbladder volume and the contraction rate decrease significantly. There were 71 cases (32. 87%)of Liver-Qi stagnation type and 9 cases (4.17%)ofLiver Yin-Deficiency type, whose characters of the gallbladder were not prominent, so the identification of these cases depended largely on clinical differentiation. Conclusions: This study wb facilitate the further extending the principle of inspection in the Specific organs which may provide us with the objective basis for the differentiation of binarytract diseases.
