益脉降压流浸膏对老年高血压病患者血小板活化、纤溶活性及血管紧张素Ⅱ的影响

目的：观察益脉降压流浸膏对老年气虚血瘀证型Ⅱ期高血压病患者活化血小板ａ－颗粒膜蛋白（ＧＭＰ－１４０）、组织型纤溶酶原激活剂（ｔ－ＰＡ）及其抑制剂（ＰＡＩ）、血管紧张素Ｈ（ＡｕｇⅡ）的影响。方法：采用放射免疫法及比色分析法测定４２例老年气虚血瘀证型Ⅱ期高血压病患者（治疗组）应用益脉降压流浸膏治疗前后ＧＭＰ－１４０、ｔ－ＰＡ、ＰＡⅠ及ＡｕｇⅡ水平；并与３０例应用卡托普利治疗的患者（对照组）及３０名老年健康人（健康对照组）进行比较。结果：治疗前两组患者较健康对照组ＧＭＰ－１４０、ＰＡＩ及ＡｕｇⅡ均显著升高（Ｐ＜０．０１），ｔ－ＰＡ显著降低（Ｐ＜０．０１）；治疗后治疗组上述指标均明显改善，与治疗前比较有显著性差异（Ｐ＜０．０５，Ｐ＜０．０１），其中ＧＭＰ－１４０、ｔ－ＰＡ、ＰＡＩ改善优于对照组（Ｐ＜０．０５，Ｐ＜０．０１），而降低ＡｎｇⅡ作用弱于对照组（Ｐ＜０．０１），治疗后两组血压值比较无明显差异（Ｐ＞０．０５）。结论：老年气虚血瘀证型Ⅱ期高血压病患者存在血小板活化增强、纤溶活性降低及血管紧张素Ⅱ水平增高；益脉降压流浸膏有较好的抑制血小板活化、增强纤溶活性作用；推测益脉降压流浸膏抑制血小板活化、增强纤溶活性及降?

To observe the effect of Yimai Jiangya extract (YMIYE) on platelet a-granule membrane protein (GMP-140), tissue-type plasminogen activator (t-PA) and its inhibitor (PAI), angiotensin Ⅱ (Aug Ⅱ ) in aged patients with essential hypertension (EH) of stage 2 caused by Qi deficiency and blood stasis. Methods: Radioimmunoassay (RIA) and colorimetric analysis were used to examine the levels of GMP-140, t-PA, PAI and Aug Ⅱ before and after treatment with YMJYE in 42 aged patients (treated group), compared with that of 30 aged patients before and after treatment with captopril (control group) and 30 aged healthy subjects (healthy group). Results: Before treatment, GMP-140, PAI and Aug Ⅱ were significantly higher (P< 0. 01), and t-PA were obviously lower (P < 0. 01 ) in the treated and the control group compared with those in the healthy group. After treatment, the improvement of GMP-140, t-PA, PAI and Ang Ⅱ in the treated group were obvious (P < 0. 05, P <0. 01 ). The improvement of GMP-140, t-PA, PAI in the treated group was more significant than that of control group (P< 0. 05, P < 0. 01 ), while the improvement of Aug Ⅱ in the treated group was worse than that of the control group (P< 0. 01 ), After treatment, there was insignificant changes about the levels of blood pressure between the treated group and the control group (P >0. 05). Conclusions: In the aged patients, GMP-140, PAI and Aug Ⅱ were increased, t-PA reduced. The effects of YMJYE in inhibiting platelet activation and improving fibrinolytic activity were better than that of captopril. These findings indicated that the mechanisms of inhibiting platelet activation and improving fibrinolytic activity and lowering blood pressure of YMJYE were correlated with decreasing the levels of Aug Ⅱ, but other mechanisms may also exist.
