复方桃仁软肝胶囊治疗慢性乙型肝炎肝纤维化患者的临床观察

目的 :观察复方桃仁软肝胶囊治疗慢性乙型肝炎肝纤维化的临床疗效。方法 :将 6 2例慢性乙型肝炎肝纤维化患者随机分为两组 ,治疗组 (32例 )口服复方桃仁软肝胶囊 ,对照组 (30例 )口服普通保肝药 ,疗程均为 6个月。观察用药前后血清肝纤维化指标、门静脉血流动力学以及肝组织病理等方面的变化。结果 :治疗组治疗后血清肝纤维化各项指标和门静脉主干血流动力学各参数均有明显改善 (P <0 0 5或P <0 0 1) ,与对照组比较 ,差异有显著性 (P <0 0 5或P <0 0 1)。有 15例 (治疗组 11例、对照组 4例 )在治疗前行肝活检 ,治疗组中有 2例治疗后行第 2次肝活检 ,对该 2例治疗前后两次肝活检组织进行光镜和电镜检查 ,显示肝细胞变性坏死减轻 ,肝纤维化程度明显下降 ,同时α 平滑肌肌动蛋白 (α SMA)免疫组织化学染色也显示治疗后α SMA阳性的肝星状细胞 (HSC)数明显减少。结论 :初步认为复方桃仁软肝胶囊对慢性乙型肝炎肝纤维化具有一定的抗肝纤维化作用。

英文摘要:

Objective: To observe the therapeutic effect of Compound Peach Kernel Softening Liver Capsule (CPKSLC) in treating patients with chronic hepatitis B caused liver fibrosis (CHB LF). Methods: The 62 patients with CHB LF observed were ranlomly divided into two groups. The 32 patients in the treated group were treated with CPKSLC orally taken, and the 30 in the control group with conventional liver protecting agents, the treatment course for both groups was 6 months. Changes of serum liver fibrosis indexes, hemodynamics of portal vein and liver pathology before and after treatment were observed. Results: Serum liver fibrosis indexes and hemodynamics of portal vein were improved significantly (P<0 05 or P<0 01) in the treated group after treatment, which were better than those in the control group significantly (P<0 05 or P<0 01). Two out of the 15 patients who had liver biopsy performed before treatment had their liver biopsy rechecked after treatment. Result of pathological examination of liver by light and electron microscopy showed that after treatment, the liver cell degeneration alleviated, degree of liver fibrosis reduced, and the smooth muscle actin (α SMA) positive hepatic stellate cells (immuno histochemical stain) also abated significantly. Conclusion: CPSKLC has definite anti fibrosis effect in treating CHB LF.
