      目的观察黄芪注射液对法洛四联症 (tetralogyofFallot,TOF)患儿根治术后免疫功能的影响。方法 4 0例TOF术后患儿分为对照组 (2 0例 ,给予常规治疗 )和黄芪组 (2 0例 ,在常规治疗的基础上加用黄芪注射液 15ml静脉滴注 ,每天 2次 ,共 2周 ) ;并观察两组患儿免疫球蛋白 (IgG和IgM )、补体 (C3、C4 )、淋巴细胞表型和细胞因子的变化。结果应用黄芪注射液后 ,异常升高的IgG、IgM、C3、C4 、CD8+和CD19+于治疗后第 1~ 2周开始下降 ,且于第 3~ 4周基本恢复到正常组水平 ;而异常下降的IgA、CD3+、CD4 +、CD4 +/CD8+、CD3+/HLA DR+和CD3+/CD16 + CD56 +于治疗后 1周开始逐渐升高 ,于治疗后 2~ 3周恢复到正常组水平。血浆和外周血单核细胞体外产生的白细胞介素 6和肿瘤坏死因子 α于治疗后 1周开始逐渐下降 ,于治疗后 3~ 4周恢复到正常组水平 ,上述指标治疗前后差值治疗组均优于对照组 (P <0 0 5或P <0 0 1)。结论黄芪注射液可明显改善TOF患儿术后的免疫功能。

      ObjectiveTo evaluate the effect of milkvetch injection (MI) on immune function of children with tetralogy of Fallot (TOF) after radical operation. MethodsForty-children with TOF were divided into two groups, the 20 in the control group treated with conventional treatment alone and the 20 in the treated group treated with conventional treatment plus 15 ml of MI every 12 hrs for 14 days. Changes of immunoglobulin, complements, lymphocyte phenotypes and cytokines were observed. ResultsIn the treated group, the abnormally increased levels of IgG, IgM, C 3, C 4, CD 8 + and CD 19 + began to lower at 1st-2nd week after treatment, and basically restored to the levels of normal at 3rd-4th week; while the decreased levels of IgA, CD 3 +, CD 4 +, CD 4 +/CD 8 + ratio, CD 3 +/HLA-DR + and CD 3 +/CD 16 +-CD 56 + raised gradually from the 1st week and restored to normal range at 2nd-3rd week. The IL-6 and tumor necrosis factor-α (TNF-α) levels in the plasma and supernatant, produced in vitro by peripheral blood mononuclear cells (PBMC) decreased gradually at 1st week and restored to the normal level at 3rd-4th weeks. The different value before and after treatment of the obove-mentioned indexes in the treated group were superior to those in the control group (P<0 05 or P<0 01). ConclusionMI could significantly improve the immune function of children with TOF after radical operation.
