      目的观察牙周基础治疗联合清热解毒中药合剂对慢性牙周炎患牙龈沟液(gingival crevicularfluid,GCF)中白细胞介素-1β(IL-1β)及肿瘤坏死因子-α(TNF-α)水平的影响。方法24例慢性牙周炎患者(34颗观察牙)分为中药组13例(17颗观察牙)和对照组11例(17颗观察牙);对照组给予牙周基础治疗,中药组在对照组基础上服用清热解毒中药合剂,疗程为8周。分别于治疗前、牙周基础治疗后2周及10周滤纸条法收集患牙GCF,并记录相关的临床指标,包括探诊深度(PD)、临床附着丧失(CAL)和探诊出血点数(BOP)。放免法检测GCF中TNF-α及IL-1β水平。结果两组牙周基础治疗后患牙GCF中TNF-α及IL-1β水平显著低于治疗前(P<0.01);同时患牙PD、CAL及BOP明显下降(P<0.01)。中药治疗后中药组TNF-α水平(μg/L)为2.5551±1.0157,明显低于同期对照组的3.4341±1.0762,中药组IL-1β水平(μg/L)为0.1335±0.0216明显低于同期对照组的0.1471±0.0169(P<0.05)。结论清热解毒中药合剂在牙周基础治疗基础上可进一步降低TNF-α及IL-1β水平,减轻牙周炎症反应。

      Objective To observe the effect of periodontal initial therapy together with Chinese herbal compound for clearing heat and detoxifcation(Abbr.TCM) on the levels of tumor necrosis factor-alpha(TNF-α) and interleukin-1beta(IL-1β) in gingival crevicular fluid(GCF) of patients with chronic periodontitis(CPD).MethodsThirty-four teeth of 24 patients with CPD were randomly assigned to the control group(17 teeth of 11 patients) and the treated group(17 teeth of 13 patients),they were treated for 8 weeks with periodontal initial therapy,but TCM was given additionally to the treated group.The GCF of the observed teeth was collected at the time before initial therapy(datum line) and at 2 and 8 weeks after treatment.At the same time,the clinical parameters,including probing depth(PD),clinical attachment loss(CAL),and bleeding on probing(BOP) were recorded,and the levels of TNF-α and IL-1β in GCF were measured by radioimmunoassay.ResultsThe levels of TNF-α and IL-1β in GCF after treatment were significantly lowered in all patients as compared with those before treatment(P<0.01),while PD,CAL and BOP showed significant reduction(P<0.01).After treatment,TNF-α and IL-1β levels in the treated group were lower than those in the control group(TNF-α: 2.5551±1.0157 μg/L vs 3.4341±1.0762 μg/L,IL-1β: 0.1335±0.0216 μg/L vs 0.1471±0.0169 μg/L;P<0.05).ConclusionTCM could enhance the effect of periodontal initial therapy in lowering TNF-α and IL-1β levels,which was attributed to reducing the inflammatory reaction of periodontal tissues.
