      目的研究人参、大黄、栝蒌角药配伍对实验新西兰兔动脉粥样硬化进程中血脂四项总胆固醇(to-tal cholesterol,TC)、甘油三酯(triglyeride,TG)、高密度脂蛋白胆固醇(high-density lipoprotein cholesterol,HDL-C)、低密度脂蛋白胆固醇(low-density lipoprotein cholesterol,LDL-C)水平及相关促炎因子细胞间黏附分子(in-tercellular adhesion molecule-1,ICAM-1)、血管细胞黏附分子(vascular cell adhesion molecule-1,VCAM-1)、高敏C反应蛋白(high sensitive C reactive protein,hs-CRP)的影响。方法 20只新西兰兔适应性饲养1周后,随机分为3组。正常组6只、模型组6只、中药组8只。中药组、模型组均给予高脂饲料,每天100g喂养6周,第7周中药组及模型组采用球囊拉伤术损伤髂动脉。术后均继续以高脂饮食喂养6周。中药组造模同时预防给药:按生晒参0.64g/(kg·d),栝蒌2.14g/(kg·d),酒大黄0.43g/(kg·d),给药容积2mL/kg,灌胃给药。动态观察各组新西兰兔血脂四项及ICAM-1、VCAM-1及hs-CRP的变化。结果第7周(术前)模型组血脂四项、中药组TC、HDL-C、LDL-C,模型组及中药组的ICAM-1、VCAM-1、hs-CRP均升高,与正常组比较差异具有统计学意义(P<0.05),中药组TC、TG、LDL-C值低于模型组,差异有统计学意义(P<0.05);第8周模型组血脂四项、ICAM-1及中药组TC、LDL-C、HDL-C、ICAM-1均高于正常组,差异具有统计学意义(P<0.05);第12周模型组血脂四项及中药组HDL-C、LDL-C高于正常组,差异具有统计学意义(P<0.05);中药组LDL-C值低于模型组,差异具有统计学意义(P<0.05)。模型组ICAM-1、VCAM-1及hs-CRP明显高于正常组及中药组,差异具有统计学意义(P<0.05),中药组hs-CRP高于正常组,差异具有统计学意义(P<0.05)。结论人参大黄瓜蒌角药配伍通过降低血脂尤其是降低TC、LDL-C的上升水平来达到延缓动脉粥样硬化进程的作用。而其降低促炎因子水平的作用与降脂和抗炎作用有关,也有可能与其改善内皮功能、抑制平滑肌增殖等途径有关。

      Objective To study the effect of triangle drugs as ginseng,Trichosanthes kirilowii Maxim,and rhubarb on the levels of blood lipids as [total cholesterol (TC),triglyeride (TG),high-density lipoprotein cholesterol (HDL-C),and low-density lipoprotein cholesterol (LDL-C)] and pro-inflammatory cytokines as intercellular adhesion molecule-1 (ICAM-1),vascular cell adhesion molecule-1 (VCAM-1),and high sensitive C reactive protein (hs-CRP) during the process of treating artherosclerosis. Methods Twenty New Zealand rabbits were randomly divided into three groups after one-week adaptive feeding,i.e. the normal control group (n=6),the model group (n=6),and the triangle drugs group (n=8). High fat diet was fed to rabbits in the triangle drugs group and the model group at the daily dose of 100 g for six weeks. Iliac artery was injured in the model group and the triangle drugs group at the seventh week using balloon injury. High fat diet was successively fed to those after surgery for six weeks. At the same time of modeling,preventive medication (at the daily dose of dry ginseng 0.64 g/kg,Trichosanthes kirilowii Maxim 2.14 g/kg,and prepared Radix et Rhizoma Rhei with wine 0.43 g/kg,with the volume of 2 mL/kg) was administered by gastrogavage to rabbits in the triangle drugs group. Changes of blood lipids levels and related pro-inflammatory cytokines were dynamically observed. Results On the 7th week (before surgery),the levels of TC,TG,HDL-C,and LDL-C in the model group,TC,HDL-C,and LDL-C in the triangle drugs group significantly increased,showing significant difference when compared with those of the normal control group (P<0.05). The levels of pro-inflammatory cytokines in the model group and the triangle drugs group were significantly higher than those in the normal control group (P<0.05). Levels of TC,TG,and LDL-C were lower in the triangle drugs group than in the model group,showing statistical difference (P<0.05). After the 8th week the levels of blood lipids and ICAM-1 in the model group,and levels of TC,LDL-C,HDL-C,and ICAM-1 in the triangle drugs group were significantly higher than those of the normal control group,showing statistical difference (P<0.05). After the 12th week levels of blood lipids in the model group,LDL-C and HDL-C in the triangle drugs group were significantly higher than those of the normal control group,showing statistical difference (P<0.05). The LDL-C level was lower in the triangle drugs group than in the model group,showing statistical difference (P<0.05). The levels of VCAM-1,ICAM-1,and hs-CRP in the model group were obviously higher than those in the triangle drugs group and the normal control group,showing statistical significance (P<0.05). The hs-CRP level was higher in the triangle drugs group than in the normal control group,showing statistical difference (P<0.05). Conclusions The triangle drugs may postpone the process of artherosclerosis by lowering blood lipids levels,especially by lowering the elevating levels of TC and LDL-C. Its roles in decreasing the level of pro-inflammatory cytokines might be associated with lipids lowering and anti-inflammation. Its roles may also be associated with improvement of the endothelial function and inhibition of the smooth muscle proliferation.
