| 对73例单纯胆囊切除术后选择性不放烟卷引流,于手术前后常规服用有"通里攻下"作用的协定中药汤剂,并以另70例常规放引流而不服用中药的病例作为对照组进行分析研究。不放引流者术后48小时发热38℃以上者占12.32%;切口全部Ⅰ期愈合;术后平均排气时间45.35小时。而对照组发热者占31.42%(P<0.01);术后7天原引流口不愈合者占34.29%(P<0.01);平均术后排气时间62.93小时(P<0.001)。对中西医结合的治疗方法,手术操作,不需要常规放引流的理由、适应症及优点,术后"通里攻下"疏通肠道的意义进行了讨论。 |

| In this paper is reported a clinical analysis of 73 cases of simple cholecystectomy by traditional Chinese medicine(using traditional Chinese drugs with the effect of"Tong Li Gong Xia"通里攻下)integrating western medicine without cigarette drainage and of 70 cases of cholecystectomy with cigarette drainage as control.The rate of postoperative fever(48th hour’s temperature was over 38℃)of the experimental group was lower than the control group(12.32%:31.42%,P<0,01);the average flatus-passing time was shorter(45.35:62.93 hours,P<0.001); intravenous fluids were less(3619:6812ml,P<0.001);the rate of healing by first intention was higher(100%:65.71%, P<0.01);the average days of hospitalization were shorter(9.65:12.20 days,P<0.01). In order to recover intestinal function as early as possible,we adopt the postoperative therapeutic method,i.e.,4 principles of early postoperative treatment:(1)Early administration of traditional Chinese drugs.(2)Early use of acupuncture.(3)Early out-of-bed activity.(4)Early commencement of a fluid diet.Naturally traditional Chinese drugs have the effect of"Tong Li Gong Xia"(purgation using cathartics to keep bowels open,to relieve stagnation,to eliminate the pathogcnetie heat factor and excrete accumulated water).Following questions are also discussed: therapeutic measures of integrated TCM-WM;operative procedures;reasons;indications and benefits of dropping routine drainage. |
