      目的 :探讨还聪丹对血管性痴呆 (VD)发病相关因素脂蛋白、载脂蛋白及血清免疫球蛋白等的影响。方法 :VD患者 5 2例 ,随机分为还聪丹治疗组 (2 7例 ,简称治疗组 )与脑复康对照组 (2 5例 ,简称对照组 )。治疗组治疗前后测定血清载脂蛋白 (apoA1、apoB10 0 、apoE) ;免疫球蛋白 (IgG、IgA、IgM )及免疫复合物 (CIC) ;并且两组均进行血清胆固醇 (TC)、甘油三酯 (TG)、高密度脂蛋白胆固醇 (HDL C)及低密度脂蛋白胆固醇 (LDL C)的检测。结果 :治疗组降低TG与TC的疗效优于对照组 (P <0 0 5 ,P <0 0 1) ;升高HDL C的作用亦优于对照组 (P <0 0 5 ) ,但两组间降低LDL C的作用差异无显著性 (P >0 0 5 )。还聪丹能明显升高血清apoA1和降低apoB10 0 水平 ,与治疗前比较差异有显著性 (P <0 0 5 ,P <0 0 1) ,但对apoE水平影响不明显 (P >0 0 5 ) ,同时能明显改善IgA、IgG和CIC水平 ,治疗后均有显著降低 (P <0 0 5 ,P <0 0 1)。结论 :还聪丹通过调节患者的脂代谢紊乱 ,起到改善血液粘稠度 ,提高脑血流量 ,改善血液携氧能力 ,从而达到改善或预防VD的发病 ;通过升高血清apoA1和降低apoB10 0 水平 ,预防或改善动脉粥样硬化 ,减少血管内皮细胞的损伤 ,是其防治血管性痴呆的一个主要作用机制之一 ;通过改善患者的免?

      Objective: To probe into the effect of Huancongdan capsule (HCDC) on the vascular dementia (VD) related factors, including lipoprotein, apolipoprotein (apo) and serum immunoglobulin. Methods: Fifty two patients of VD were divided into the HCDC group (27 patients, treated with HCDC) and the control group (25 patients, treated with Naofukang). Changes of related parameters were observed. Results: The effects in lowering triglyceride and total cholesterol and elevating high density lipoprotein cholesterol in the HCDC group were superior to those in the control group (P<0 01 or P<0 05), but the effect in lowering low density lipoprotein cholesterol was similar in the two groups (P>0 05). Compared with data before treatment, in the HCDC group after treatment, the levels of serum apoA 1 increased, and serum apoB 100 , IgA, IgG and circulating immune complex decreased significantly (P<0 05 or P<0 01), but the change of apoE level was insignificant (P>0 05). Conclusion: Through adjusting lipid metabolism, HCDC could improve blood rheologic characteristics, promote cerebral blood flow, improve the oxygen carrying capacity of blood to prevent and treat VD. One of its mechanisms is increasing apoA 1 and decreasing apoB 100 levels, preventing and improving atherosclerosis and reducing damage of vascular endothelial cells, the another may be its action in improving immune function.
