      目的:联合使用中药验方补肾健脾方与低剂量促红细胞生成素(huEPO,益比奥)治疗肾性贫血(RA),并观察患者血清肿瘤坏死因子-α(TNF-α)水平的变化以探讨中西医结合治疗肾性贫血的可能机制。方法:已经确诊为RA的透析患者随机分为两组,补肾健脾方与益比奥治疗组(简称中西组),单用益比奥治疗组(对照组),两组均予透析、补充铁剂、叶酸及维生素B12及对症支持治疗。中西组予益比奥50IU/kg皮下注射,每周2次;并口服补肾健脾方,每天1剂,分2次服;对照组单纯用益比奥50IU/kg皮下注射,每周3次;疗程均为3个月。治疗前后均检测血红蛋白(Hb)、红细胞压积(Hct)、TNF—a,并观察疗效。结果:(1)两组治疗后Hb、Hct均较治疗前明显上升(P<0.01),两组治疗3个月Hb、Hct比较差异有显著性(P<0.05)。(2)两组治疗前血清TNF-α均明显高于正常;治疗后,中西组血清TNF-α下降,与治疗前比较差异有显著性(P<0.05);但对照组治疗前后血清TNF-α水平无明显变化。结论:补肾健脾方合益比奥可明显抑制血清TNF-α的产生,可能是中西组有效改善EPO敏感性的重要原因。

      Objective: To study the effect of treatment of renal anemia by combination of Bushen Jianpi Recipe (BJR, a Chinese experienced recipe for supplementing Shen and supporting Pi) and low dosage erythropoietin (EPO), and the influence of treatment on change of serum tumor necrosis factor α (TNF-α) so as to explore the possible mechanism of integrative Chinese and western medicine (ICWM) in treating renal anemia. Methods: Patients with renal anemia were randomly divided into two groups, the ICWM group and the control group, symptomatic and supporting treatment such as dialysis, supplementing of ferrous, foliac acid and vitamin B12, was given to both groups. Additionally, to the ICWM group, 50 IU/kg of EPO subcutaneous injection for twice every week, and oral intake of BJR, one dose per day taken in two parts, were given, and to the control group, EPO alone, 50IU/kg by subcutaneous injecting, 3 times per week, was given. The therapeutic course for two groups was 3 months. Blood levels of hemoglobin (Hb) , hematocrit (Hct) , TNF-α were measured before treatment and the therapeutic effect was observed. Results: After treatment, the levels of Hb and Hct increased significantly in both groups (P<0.01), comparison between the two groups in Hb and Hct after being treated for 3 months showed significant difference ( P < 0.05). The serum level of TNF-α was markedly higher than normal range in both groups before treatment, it significantly lowered after treatment in the ICWM group (P <0.05 ) , but unchanged in the control group. Conclusion: Combination of BJR and EPO could significantly inhibit the production of TNF-α, this may be an important factor for ICWM in effectively improving sensitivity to EPO.
