      目的探讨苦参素联合干扰素α-1b(interferonα-1b,IFNα-1b)治疗慢性乙型肝炎(chronichepati-tisB,CHB)患者的临床疗效。方法将146例CHB患者随机分为A、B、C、D4组。在常规保肝治疗(D组)基础上,A组加用苦参素联合IFNα-1b治疗;B组加用干扰素α-1b;C组加用苦参素,疗程均为6个月,随访6个月。采用免疫组织化学检测、放射免疫法检测及酶联免疫吸附法,对治疗前后肝组织学、肝组织转化生长因子-β(TGF-β)的表达及血清TGF-β、透明质酸(HA)、层粘连蛋白(LN)、Ⅳ型胶原(Ⅳ-C)、Ⅲ型前胶原(PCⅢ)指标变化及HBeAg、HBVDNA转阴率及ALT复常率等进行了观察。结果治疗后A组组织学肝纤维化程度计分、肝组织TGF-β的表达及血清TGF-β、HA、LN、Ⅳ-C、PCⅢ及HBeAg、HBVDNA转阴率等指标值均显著下降,与B、C组比较,差异均有显著性(P<0·05,P<0·01),肝组织TGF-β转阴率的表达与肝脏纤维化的动态变化一致,B组与C组比较差异无显著性。结论苦参素联合IFNα-1b治疗慢性乙型肝炎的疗效优于单一用药。

      ObjectiveTo study the clinical effect of combination of kurarinone (KR) and interferon α-1b (IFNα-1b) in treating chronic hepatitis B. MethodsOne hundred and forty-six patients with chronic hepatitis B were randomized into four groups. Under the basic conventional treatment, additional KR combined IFNα-1b was given to Group A, IFNα-1b to Group B and KR to Group C while none was given to Group D. The therapeutic course for them was 6 months and succeeded with 6 months of follow-up. Changes in liver histology, expression of transforming growth factor β(TGF-β) in liver tissue, and serum levels of TGF-β, hyaluronic acid (HA), laminin (LN), collagen type Ⅳ (ⅣC), precollagen Ⅲ (PCⅢ), as well as the negative conversion rate of HBeAg and HBV DNA, and normalization rate of alanine transaminase (ALT) before and after treatment were observed by immuno chemical method, RIA and ELISA. ResultsAfter treatment, in Group A, the scores of liver fibrosis and all the above-mentioned indexes significantly lowered, as compared with those in Group B and C, the difference was significant (<0.05 or <0.01). The negative conversion rate of TGF-β in liver tissue was in accordance with the dynamic change of liver fibrosis. Comparison between Group B and C in those aspects showed insignificant difference (>0.05). ConclusionKR combined with IFNα-1b shows better effect in treating chronic hepatitis B than that of using either of the two alone.
