      目的动态观察中药复方肾衰合剂对急性肾功能衰竭(ARF)大鼠肾脏修复的影响。方法雄性SD大鼠分为4组,除正常组外,其余大鼠均予肌肉注射甘油造成ARF模型,同时正常组、模型组予生理盐水,西药组予维拉帕米、中药组予肾衰合剂灌胃。观察各组大鼠1~5天肾功能、肾脏病理、肾组织表皮细胞生长因子(EGF)mRNA、增殖细胞核抗原(PCNA)表达的变化。结果(1)肾功能:模型组及两用药组第1天时血肌酐(SCr)、尿素氮(BUN)显著增高(与正常组比较,P<0·01),但中药组、西药组大鼠SCr上升幅度较模型组低(P<0·05);中药组大鼠第3天时肾功能优于模型组和西药组(P<0·01),至第5天时接近正常组。(2)肾脏病理:中药组大鼠各期肾组织病变均较模型组轻,尤其在第3天时(约中期)可见部分肾小管细胞再生,至第5天(约恢复期)肾组织重建良好,且明显优于西药组。(3)免疫组化及原位杂交:中药组大鼠第3天时肾小管上皮细胞EGFmRNA、PCNA的表达显著增高,与同期其他组比较差异有显著性(P<0·05)。结论肾衰合剂通过促进肾小管上皮细胞EGFmRNA的表达,促进肾脏组织的再生修复,缩短了ARF的病程。

      ObjectiveTo dynamically observe the effect of Shenshuai Mixture (SM ) on repair of kidney in acute renal failure (ARF) rats. Methods Male SD rats w ere divided into 4 groups randomly, the normal group, the SM group, the verapami l group and the model control group. Except those in the normal group were trea ted with normal saline without modeling, all remaining rats, after being made in to ARF model by intra-muscular injection of glycerin, were treated with SM, v e rapamil and normal saline respectively via gastrogavage. Renal function, renal p athology, mRNA expression of epidermal growth factor (EGF) and protein expressio n of proliferating cell nuclear antigen (PCNA) were detected once every day from the 1th day to the 5th day. Results(1) BUN and Scr lev els increased markedly 2 4 hrs after modeling, but the Scr level in the two treated groups was significan tly lower than that in the model group (P<0.05). Compared with that in the model group and the verapamil group, renal function was better in the SM group on th e 3rd day (P<0.01), and approach to normal level on the 5th da y. (2) Renal patho logical changes alleviated in every phase of ARF in the SM group than that in th e model group, especially part of tubule regeneration could be seen on the 3rd d ay (metaphase), and renal structure was rehabilitated on the 5th day (convalescence ), prior to those in the model group. (3) At the 3rd day expression of EGF mRNA an d PCNA in tubule epithelial cell (TEC) increased remarkably in the SM group, highe r than those in the model and verapamil group (P<0.05). ConclusionSM could pro mote renal tissue regeneration and rehabilitation, and shorten the course of ARF through up-regulating mRNA expression of EGF in TEC.
