      目的探讨通冠胶囊对冠心病患者经皮冠状动脉介入(percutaneous coronary intervention,PCI)术后外周血内皮祖细胞(endothelial progenitor cells,EPCs)数量的影响。方法选择2010年3—10月广东省中医院ICU和心脏中心收治的冠心病行PCI术患者60例,按照随机数字法分为通冠胶囊治疗组(治疗组)和安慰剂对照组(对照组),每组30例。手术当天开始服用通冠胶囊或安慰剂3粒,每天3次,连续3个月。检测PCI术前及术后1个月患者外周血CD34 及血管内皮生长因子受体2(VEGFR2)阳性细胞数目。于PCI术前和术后3个月行超声心动图检查,测定左室舒张末容积(LVEDV)、左室收缩末容积(LVESV)、每搏输出量(SV)、每分输出量(CO)、左室射血分数(LVEF),评价两组患者“壁运动指数”(wall motion score index,WMSI)。结果术前两组患者EPCs数量、LVEF、WMSI及SV比较,差异无统计学意义(P>0.05),两组术后1个月较术前外周血中EPCs数量均有所上升(P<0.05),且治疗组明显高于对照组,差异有统计学意义(P<0.05);术后3个月两组患者LVEF升高(P<0.05,P<0.01),治疗组WMSI降低(P<0.05),SV 升高(P<0.05),治疗组LVEF及WMSI改善程度优于对照组(P<0.05)。 结论通冠胶囊可以上调冠心病患者PCI术后外周血EPCs的数量,改善患者的心功能。

      ObjectiveTo observe the effect of Tongguan Capsule (TC) on the number of endothelial progenitor cells (EPCs) in the peripheral blood of patients with coronary artery disease (CAD) after percutaneous coronary intervention (PCI). MethodsRecruited were 60 CAD patients undergoing PCI who were admitted and treated at ICU and the Heart Center of Guangdong Provincial Hospital of Traditional Chinese Medicine from March to October 2010. They were assigned to the treatment group (treated by TC) and the control group (treated by placebos) according to the random digit table, 30 cases in each group. They took TC or placebos from the day of PCI, three pills each time, three times a day, for three consecutive months. The numbers of peripheral blood CD34 and vascular endothelial growth factor receptor 2 (VEGFR2) positive cells were detected before PCI and 3 months after PCI respectively. The echocardiography was performed before PCI and 3 months after PCI respectively to determinate the left ventricular end diastolic volume (LVEDV), left ventricular end systolic volume (LVESV), stroke volume (SV), cardiac output (CO), and left ventricular ejection fraction (LVEF). The wall motion score index (WMSI) was assessed in the two groups. ResultsThere was no statistical difference in the number of EPCs, LVEF,WMSI, or SV in the two groups before PCI (P>0.05). The number of EPCs increased in both the two groups after 1 month of PCI (P<0.05). It was obviously higher in the treatment group than in the control group (P<0.05). The LVEF both increased in the two groups 3 months after PCI (P<0.05, P<0.01). The WMSI decreased and SV increased in the treatment group (P<0.05). The improvement of LVEF and WMSI was better in the treatment group than in the control group (P<0.05). ConclusionTC could up regulate the number of EPCs in the peripheral blood of CAD patients after PCI, and improve their cardiac functions.
