      目的 观察针刺与艾灸改善哮喘大鼠肺功能及气道炎症的作用,并比较疗效差异。方法 将48只SD大鼠随机分为正常组、模型组、针刺组和艾灸组,每组12只。采用卵清白蛋白致敏和雾化激发制备哮喘大鼠模型。造模成功后,针刺组和艾灸组大鼠分别给予双侧“肺俞”穴针刺与艾灸干预30 min,每天1次,连续14天。干预结束后,使用肺功能测定仪测定大鼠肺功能[最大呼气流量(PEF)、肺阻力(RL)、动态肺顺应性(Cdyn)、呼出25 %肺活量时最大呼气流量(FEF 25 %)、用力呼气量占用力肺活量比(FEV/FVC)、最大呼气中期流量(MMEF)];分别经HE和Masson染色后,光镜下观察大鼠肺组织的病理形态学变化及胶原沉积程度;运用酶联免疫吸附法检测各组大鼠肺组织中半胱氨酸白三烯(LT)、白介素(IL)-17、IL-4、IL-13、IL-33、IL-5、胸腺基质淋巴细胞生成素(TSLP)、前列腺素D2(PGD2)表达。结果 与正常组比较,模型组PEF、Cdyn、FEF 25 %、FEV/FVC、MMEF下降,RL、胶原纤维沉积、肺组织中LT、IL-17、IL-4、IL-13、TSLP、IL-33、PGD2、IL-5表达增加(P<0.05,P<0.01)。与模型组比较,针刺组大鼠FEF 25 %、FEV/FVC增加,胶原纤维沉积、肺组织LT、IL-17、IL-4、IL-13、TSLP、IL-33、PGD2、IL-5表达降低(P<0.05,P<0.01);艾灸组大鼠Cdyn、FEF 25 %、MMEF增加,胶原纤维沉积、肺组织IL-17、IL-4、IL-13、TSLP、IL-33、IL-5表达降低(P<0.05,P<0.01)。与艾灸组比较,针刺组大鼠肺组织中LT、IL-13、PGD2、IL-5表达降低(P<0.05,P<0.01)。结论 针刺与艾灸“肺俞”穴均能改善哮喘模型大鼠的肺组织损伤、肺功能及气道炎症;且两者在改善肺功能和气道炎症的相关指标上存在一定差异性。

      Objective To observe the effect of acupuncture and moxibustion on lung function and airway inflammation in asthma rats,and to compare and observe the difference in curative effect. Methods Totally 48 SD rats were randomly divided into normal group,model group,acupuncture group and moxibustion group,12 rats in each group. Except the normal group,the asthma model was established by ovalbumin sensitization and aerosol challenge. After the model was established,rats in the acupuncture and moxibustion groups were treated with acupuncture and moxibustion at bilateral "Feishu" (BL 13) points for 30 min respectively,one time per day,the course was 14 consecutive days. After the end of the intervention,the pulmonary function of the rats was measured by pulmonary function analyzer,included peak expiratory flow(PEF),lung resistance(RL),dynamic lung compliance(Cdyn),forced expiratory flow 25 %(FEF 25 %),forced expiratory volume / forced vital capacity(FEV/FVC),maximal mid-expiratory flow(MMEF). The pathological changes and the degree of collagen deposition in the lung tissue of the rats were observed by light microscope after HE and Masson staining respectively. The levels of leukotrienes (LT),interleukin(IL)-17,IL-4,IL-13,IL-33,IL-5,thymic stromal lymphopoietin (TSLP) and prostaglandin D2(PGD2) in the lung tissues of rats were measured by Elisa. Results Compared with the normal group,PEF,Cdyn,FEF 25% ,FEV/FVC,MMEF decreased,RL,deposition of collagen fibers and lung tissue LT,IL-17,IL-4,IL-13,TSLP,IL-33,PGD2,IL-5 expression increased(P<0.05,P<0.01). Compared with the model group,FEF 25% and FEV/FVC increased,deposition of collagen fibers,lung tissue LT,IL-17,IL-4,IL-13, TSLP,IL-33,PGD2 and IL-5 expression in the acupuncture group(P<0.05,P<0.01). The Cdyn,FEF 25% and MMEF increased,deposition of collagen fibers,lung tissue IL-17,IL-4,IL-13,TSLP,IL-33,IL-5 expression decreased in the moxibustion group(P<0.05,P<0.01). Compared with the moxibustion group,lung tissue LT,IL-13,PGD2 and IL-5 expression decreased in the acupuncture group(P<0.05,P<0.01). Conclusions Acupuncture and moxibustion "Feishu" (BL 13) acupoint could improve lung tissue damage,lung function and airway inflammation in asthmatic rats. Moreover,there are some differences in the dominant indicators of improving lung function and airway inflammation.
