      目的 探讨2型糖尿病肾病代谢综合征(MetS)患者中医证素与内脏脂肪指数的相关性。方法 收集2021年3—11月在中国人民解放军总医院第一医学中心肾脏病医学部住院的187例2型糖尿病肾病患者的中医问卷、临床资料及内脏脂肪指数(VAI)指标,根据MetS的诊断标准将2型糖尿病肾病患者分为MetS组150例和无MetS组37例。观察所有2型糖尿病肾病患者中医症状、证素及VAI指标,两组患者中医病位和病性证素比较采用Student 's t检验和卡方检验或Mann-Whitney U检验。取单因素线性回归分析中P<0.05的指标为自变量进行多重线性回归分析。因为VAI的计算公式包含了甘油三酯和高密度脂蛋白,进行自变量纳入时,未纳入这两个指标。结果 与无MetS组比较,MetS组的体重指数(BMI)、腰围(WC)、VAI、总蛋白、血小板、甘油三酯、触珠蛋白较高(P<0.01,P<0.05),高密度脂蛋白降低(P<0.01);其中MetS 组中医病性证素发生频率从高到低分别是血瘀证、血虚证、阴虚证、热盛证、阳虚证、气虚证、水湿证、湿阻证、痰浊证、气滞证,中医病位证素发生频率从高到低则分别是肾虚证、心虚证、肝虚证、脾虚证、肺虚证;通过单因素线性回归分析发现VAI与年龄、糖化血红蛋白、空腹血糖、总胆固醇、甘油三酯、高密度脂蛋白、痰浊证、气滞证之间均存在线性关系(P<0.05),将单因素线性回归分析筛选的指标进行多重线性回归分析发现空腹血糖(P<0.01)、痰浊证(P=0.003)、气滞证(P=0.021)是2型糖尿病肾病MetS患者VAI的相关因素。结论 空腹血糖、痰浊证、气滞证是2型糖尿病肾病MetS人群VAI指标的相关因素。

      Objective To explore the correlation between Chinese medicine(CM) syndrome elements and visceral adiposity index(VAI) in type 2 diabetes patients with nephrotic metabolic syndrome(MetS).Methods The CM questionnaires,clinical data and VAI index of 187 patients with type 2 diabetic nephropathy who were hospitalized in the Department of Nephrology,the First Medical Center of the Chinese People's Liberation Army General Hospital from March 2021 to November 2021 were collected.The patients with type 2 diabetic nephropathy were assigned into MetS group(150 cases) and without MetS group(37 cases).CM symptoms,syndrome elements and VAI indexes of all patients with type 2 diabetic nephropathy were observed.CM disease location and syndrome elements of the two groups were compared by Student's t test,Chi-square test or Mann-Whitney U test.In the single factor linear regression analysis,the index with P< 0.05 was taken as the independent variable for multiple linear regression analysis.Since the calculation formula of VAI includes triglyceride and high-density lipoprotein,these two indexes were not included when included independent variables.Results Compared with the non-MetS group,the MetS group showed higher body mass index(BMI),waist circumference(WC),VAI,total protein,platelets,triglycerides,and haptoglobin(P<0.01,P<0.05),and lower high-density lipoprotein levels(P<0.01).The frequency of CM syndrome elements in MetS group from high to low were blood stasis syndrome,blood deficiency syndrome,yin deficiency syndrome,heat excess syndrome,yang deficiency syndrome,qi deficiency syndrome,water-dampness syndrome,dampness obstruction syndrome,phlegm turbidity syndrome,and qi stagnation syndrome,and the frequency of CM disease location syndromes from high to low were Shen deficiency syndrome,Xin deficiency syndrome,Gan deficiency syndrome,Pi deficiency syndrome,and Fei deficiency syndrome,respectively.Through single factor linear Regression analysis found that there was a linear relationship between VAI and age,glycated hemoglobin,fasting blood glucose,total cholesterol,triglyceride,high-density lipoprotein,phlegm turbidity syndrome,and qi stagnation syndrome(P<0.05).Multiple linear regression analysis was performed on the indicators screened by single factor linear regression analysis it was found that fasting blood glucose (P<0.01),phlegm turbidity syndrome(P=0.003), and qi stagnation syndrome(P=0.021) were the related factors of VAI in people with type 2 diabetic nephropathy and metabolic syndrome.Conclusion Fasting blood sugar,phlegm turbidity syndrome,and qi stagnation syndrome are the related factors of VAI index in people with type 2 diabetic nephropathy and metabolic syndrome.
