      目的 观察稳心颗粒对急性冠脉综合征(acute coronary syndrome,ACS)患者血浆脑钠肽(brain natriuretic peptide,BNP)及心率变异性(heart rate variability,HRV)的影响。方法 65例ACS患者随机分为治疗组(35例)和对照组(30例)。两组均给予常规治疗,治疗组加服稳心颗粒9 g,每天3次,共治疗90天。治疗前及治疗结束后分别测定患者血浆BNP及HRV参数。结果 两组治疗前BNP比较,差异无统计学意义(P>0.05);治疗后两组BNP均较本组治疗前显著下降(P<0.05),以治疗组下降更为明显(P<0.05)。两组治疗前HRV参数比较,差异无统计学意义(P>0.05);与本组治疗前比较,治疗后两组相邻RR间期差值的均方根(RMSSD)、相邻两个RR间期>50 ms的个数占所分析信息间期内RR间期总个数百分比(PNN50%)及高频功率(HF)明显增高,低频功率(LF)及低频/高频谱功率比值(LF/HF)显著下降,差异均有统计学意义(P<0.05),且治疗组上述指标均明显优于对照组(P<0.05)。结论 在常规治疗基础上加用稳心颗粒可能通过改善心肌缺血而下调ACS患者血浆BNP水平,调节HRV,从而改善近期临床预后。
英文摘要:
      Objective To observe the effect of Wenxin Granule (WG) on brain natriuretic peptide (BNP) and heart rate variability (HRV) of acute coronary syndrome (ACS) patients. Methods Totally 65 ACS patients were randomly assigned to the treatment group (35 cases) and the control group (30 cases). All patients were treated with routine therapies such as angiotensin coversing enzyme inhibitors (ACEI) and metoprolol. Those in the treatment group took WG, 9 g each time, three times daily. All were treated for 90 days. Plasma samples of BNP and HRV were determined before treatment and after treatment. Results There was no statistical difference in pre treatment plasma BNP (P>0.05). Plasma BNP significantly decreased after treatment in the two groups when compared with before treatment (P<0.05). The decrease was more obvious in the treatment group (P<0.05). There was no statistical difference in pre treatment HRV (P>0.05). Compared with before treatment in the same group, RMSSD, PNN50%, and high frequency (HF) obviously increased, while low frequency (LF) and LF/HF ratio significantly decreased in the two groups, showing statistical difference (P<0.05). The aforesaid indicies were obviously better in the treatment group than in the control group (P<0.05). Conclusion Additional administration of WG could improve short term clinical prognosis by down regulating plasma BNP level (via improving myocardial ischemia) and modulating HRV.
